The History of Breast Implants

Prior to the development of prosthetic breast implants numerous materials were trialed for the purpose of augmenting the female breast. Up until the 1950's materials included autogenous fat and dermal grafts, fat injections, paraffin injections, insertion of glass balls, ivory, rubber, and terylene wool.4,5 These materials frequently led to infection, tissue necrosis and firmness of the breast. The autogenous materials were uniformily troubled by resorption. The 1950-1960's saw the use of many other products. Free injections of hydrocarbons, petroleum jelly, silicone oils, vegetable oils and bees wax were tried and led to many problems including infection, granulomas, fistulas and breast firmness.4,5 During this same time many different types of sponges were developed hoping that soft tissue ingrowth would improve the biocompatibility of these products. Polyvinyl and polyether were the most common sponge materials (figure 2) others included silastic and Teflon. Unfortunately these products were complicated by many of the same problems and were discontinued.
The development of the silicone gel prosthesis in 1962 marked a very important new era in breast surgery. Since Cronin and Gerow6 first reported its use very few other materials have been used for breast augmentation. This is partly due to the success of silicone devices and also by the subsequent FDA regulation of medical devices that was started shortly after the development of silicone gel devices.

Two Types of Breast Implant Placement

When the implant is placed sub muscularly under the breast, there is a lower chance of contracture (contraction of the tissue capsule surrounding the implant), and mammography is more reliable. There is also less risk of visible or palpable implant edges. However, in some cases the implant has a more natural appearance when placed above the muscle, under the breast tissue. Two Different Types of Breast Implants: Saline vs. Silicone

This is because the rupture of a silicone implant may release the silicone material into the body's tissues which is a significant health concern. One of the most popular forms of treatment for reducing the appearance of fine lines and wrinkles and create smoother, younger skin is injections. In an office-based procedure, the physician uses fine needles to insert a filler into the skin to add volume, replace collagen and/or stimulate natural collagen and new skin cell growth. BOTOX Injections (Botulinum Toxin)

Botulinum toxin injections (BTX), often referred to by the product names BOTOX® Cosmetic or Myobloc®, are biological toxins (botulinum toxin) transformed into a therapeutic agent. Work with BOTOX began in the late 1960s to treat neurological disorders. Today, BOTOX is used for the treatment of frown lines, forehead furrows, "crow's feet," lines and wrinkles of the lower face, and even nasal muscles (to decrease nostril flaring). BOTOX injections have proven to be a very popular nonsurgical cosmetic procedure; see current American Society for Aesthetic Plastic Surgery (ASAPS) statistics. Aesthetic plastic surgeons have found that the type of lines and wrinkles that respond to BOTOX injections are those caused by the muscles-specifically those muscles that contract during facial expressions such as frowning or squinting.

Technique
The patient is asked to contract the muscles in the area being treated so the surgeon can determine the proper location for injection. In most cases, BOTOX is injected directly into the muscle with a tiny needle. It takes a few days to realize the effect of BOTOX injections.

Benefits

  • No downtime. Patients can immediately resume normal activities.
  • Long experience has proven BOTOX to be safe. To date, no systemic complications associated with BOTOX injections have been documented.
  • Treatment is reversible within several months.
  • May be beneficial for treatment of migraine headaches.

Other Considerations

  • BOTOX injections are a temporary solution for the treatment of wrinkles. The effects last from 3-4 months and require repeat treatments.
  • Possible side effects include local numbness, swelling, bruising, or a burning sensation during injection. Some patients have reported temporary headache and nausea. Most complications are of short duration and can be avoided with proper injection techniques.
  • A small percentage of patients are reported to experience no improvement at all.

ASAPS Position:
The American Society for Aesthetic Plastic Surgery (ASAPS) maintains that BOTOX injections for aesthetic purposes appear to be safe and effective. Patients who show early signs of aging, as well as those who may not be suitable candidates for more extensive aesthetic facial surgery, may be good candidates for this procedure. Certain medications (some antibiotics, anti-inflammatories, or aspirin) and even some vitamins and herbs may increase the potency of BOTOX and may increase bleeding and bruising at the time of injection. Therefore, patients should be candid with their surgeon about all medications and supplements. Pregnant or nursing women should postpone undergoing this procedure. It is not known whether injection of BOTOX has any effect on a fetus or whether it is found in breast milk.
